Effective March 25, 2020
Prior to your New Patient Virtual Appointment being scheduled, you must:
1. Fill out a new patient packet (see link below) and return it completely filled out by taking a picture of it or scanning it and emailing it back to us (email@example.com) or texting it through our Klara secure messaging system (see link below).
2. In the same manner, send a picture of your insurance card (FRONT AND BACK) and a picture of a Picture ID, such as a driver's license.
3. You must also reply “YES” to the Consent for Virtual Exam that will be texted to you through Klara, our secure messaging system.
Once all of these steps are completed, our staff will contact you to secure your appointment time for your New Patient Virtual Telemedicine Visit, as well initiating the set up for the application, "PocketPatient", that will be used for communication. They will also be collecting any copay or payment for your visit.
