New patient forms

New patient information can be entered in your patient portal, completed upon arrival to clinic, or printed and completed prior to your arrival. We must have a copy of a government-issued identification card to verify each patient's identity and a copy of your insurance card(s) to file your insurance.

Communication

Our clinic is open Monday to Thursday from 8:00am until 5:00pm, and on Friday from 8:00am until 12:00pm. You may reach us by phone, email, secure text, or by a direct message from your patient portal. Please allow 48 hours for us to respond to any message or refill request.

Payment and Insurance

We accept most insurances and will file it as a courtesy to you.  Your insurance plan determines whether you pay all or part of your visit charges. It is our policy to collect any co-payment, co-insurance, and/or deductible at the time of service. It is your responsibility to understand your insurance benefits (co-payment, co-insurance, deductible), network status, and covered services as these differ for every patient and their insurance plan. If you receive cosmetic or aesthetic services, full payment is required at the time of service as these services are not covered by insurance. Please see our "INSURANCE" page for more information. 

No-show, Cancellation and Late Policy

If any scheduling conflicts arise, we are happy to reschedule your appointment to a time that is more convenient for you. Please be advised that we require at least a 24-hour notice to cancel or reschedule an appointment. Failure to make your appointment ("no-show") or cancellations or rescheduled appointments made less than 24 hours prior to your appointment time will be charged a $50.00 fee. We ask that arrive early for your scheduled appointment to complete forms or update contact or insurance information. Patients arriving more than 15 minutes (grace period) after their scheduled appointment time may have to be rescheduled.

Pathology

If you have a biopsy or procedure performed, all specimens removed will be sent to an independent lab, Skin Diagnostics or Dermatopathology Associates, for examination by a pathologist. The pathology lab will process and file your insurance also. You will receive a separate bill from the pathology lab for any amount not covered by your insurance. Magnolia Dermatology has no control of their billing.

Laboratory

If you have blood drawn it will be sent to a separate, independent laboratory, Labcorp, for processing. Magnolia Dermatology is not affiliated with Labcorp and no control over their billing. Labcorp will process and file your insurance. You will receive a separate bill from Labcorp for any amount not covered by your insurance
